1. I hereby give and grant to the Forest County Potawatomi Community Wisconsin and its business enterprise Potawatomi Bingo Casino (Casino), and their respective licensees, successors and assigns (herein collectively called the licensed parties"), the right to use, publish and copyright my picture, portrait, and likeness obtained in my participation in the Event described herein for use in the Expansion promotional video and any future campaigns in the advertising and promotion of Potawatomi Bingo Casino, as follows:

Term of Use ("Term"): In Perpetuity

Scope of Use: All advertising media including but not limited to broadcast, television and radio, print, direct mail, magazine, billboard, online, internet, etc.

Territory: Throughout the Universe

2. I agree that all photographs, film video, and audio recordings and or written materials of me used and taken by the licensed parties are owned by them and that they may copyright material containing same. If I should receive any print, negative or other copy thereof, I shall not authorize its use by anyone else.

3. I agree that no advertisement or other material need be submitted to me for approval and waive any right to notice or approval of any use of the photographs or recordings used by Casino.

4. I agree that the licensed parties shall be without liability to me for any distortion or illusionary effect resulting from the publication of my picture, portrait or likeness.

5. Nothing herein will constitute any obligation on the licensed parties to make any use of the rights set forth herein.

BY SIGNING BELOW I ACKNOWLEDGE THAT I AM AT LEAST 18 YEARS OF AGE, HAVE READ AND UNDERSTAND THIS AGREEMENT AND HAVE THE AUTHORITY TO AGREE TO AND GRANT THE RIGHTS SPECIFIED HEREIN.
